A more complete history, ready before the first visit.
A tech-enabled virtual specialty clinic with an interdisciplinary care team for complex chronic conditions.
Complex histories arriving as a folder of photos.
Before a member's first visit, the care team asks them to send their labs. Today those arrive as photos and PDFs from all different sources, uploaded by hand into the EHR with no structured ingestion, on top of everything else onboarding already asks of a new member.
These are complex chronic patients who have seen many doctors, so their history is spread far and wide over many years. Assembling it is hard to solve manually, and the next set of conditions the clinic is taking on is a level up in complexity.
One link, a more complete record, at onboarding.
A single patient-authorized link runs the full flow: identity verification, retrieval across health information exchange networks, direct EHR logins, and voice and browser agents for the long tail. Records come back as structured FHIR or PDF, straight into the clinic's EHR.
It is modular, so the clinic can start with exchange and direct-login retrieval, measure coverage, and add voice agents for the remaining gap. The link goes out before the first appointment and hands the patient back to the clinic's own app.
What Hubble delivered

- HIE retrieval plus direct EHR logins
- Voice and browser agents for the long tail
- Structured FHIR or PDF, into the existing EHR
- Modular, so coverage can be measured then expanded
What changed after Hubble.
